Although informed consent is important in clinical research, questions persist regarding when it is necessary, what it requires, and how it should be obtained. The standard view in research ethics is that the function of informed consent is to respect individual autonomy. However, consent processes are multidimensional and serve other ethical functions as well. These functions deserve particular attention when barriers to consent exist. We argue that consent serves seven ethically important and conceptually distinct functions. The first four functions pertain (...) principally to individual participants: (1) providing transparency; (2) allowing control and authorization; (3) promoting concordance with participants' values; and (4) protecting and promoting welfare interests. Three other functions are systemic or policy focused: (5) promoting trust; (6) satisfying regulatory requirements; and (7) promoting integrity in research. Reframing consent around these functions can guide approaches to consent that are context sensitive and that maximize achievable goals. (shrink)

I argue for the authenticity of a fragment found in Eusebius, PE i 2.2-5, and sometimes attributed to Porphyry of Tyre. I argue against the case for non-Porphyrian authorship that has become dominant in recent years, employing evidence that highlights congruity with generally accepted Porphyrian works. This allows me to move on to an initial reconstruction of Porphyry’s religious epistemology, and to assess what that means in his historical context.

Many people lack autonomy because they work jobs that deny them significant and meaningful control over what they do. The negative impact of this can be ameliorated, to a degree, by the relationships that people often form with co-workers: that is, workplace sociability can itself enhance workers’ autonomy while also helping them tolerate heteronomous work by making it more bearable. In addition, workplace sociability is also a potential resource for advancing the cause of working people’s autonomy, acting as a basis (...) for developing forms of workplace solidarity which workers can then use, through strike action and other forms of militant activity, to improve their working conditions. In this paper, we identify the tension between, on the one hand, the ameliorative andtherapeuticvalue of workplace sociability and, on the other, sociability’s potentialinstrumentalrole in expanding workers’ autonomy. We argue that developing workplace sociability into more purposive forms of solidarity involves putting at risk those other functions performed by such sociability. If expanding their autonomy is something workers have reason to care about, navigating the varied and complex functions performed by workplace sociability is thus an important dimension of workplace organizing. ‘Everyone who now reads and writes in the West, of whatever racial background, sex or ideological camp, is still a son or daughter of Homer.’ While the extent to which this claim is accurate has been disputed, it is not wrong in our own day to grant the highest honours for ongoing influence to the author of theIliad. All the more so in Late Antiquity, a period frequently viewed as hermetically isolated from the classical world, but which resolutely viewed itself (...) as part of that unbroken cultural and literary continuum. One of those who made repeated use of Homer's epic was the Emperor Julian, one of the most prolific writers among Rome's emperors. In the fourth centurya.d., Homer's influence was still predominant, not only being Julian's favourite and most frequently cited author but also forming for Libanius of Antioch ‘one of the pillars of rhetorical teaching’. Despite Glen Bowersock's statement that Julian's many writings offer unique insight into his character and disposition, Julian is still a historical character who is not easy to ‘know’. Julian's life was shaped by the murder of his father, brothers and uncles by a cabal involving, if not orchestrated by, his cousin Constantius II. This was followed by the removal of his trusted confidant Salutius, again by Constantius. These experiences exhibit an unusual phenomenon, in that, when Julian referred to them, they were prefaced by a spate of Homeric allusions. Julian's wrath at people taken from him was both genuine and politically useful, but the expression of it was dangerous enough that he expressed it obliquely in the language of Homer. These citations and allusions, drawn primarily from theIliad, were far more than Julian's flaunting of his education, but were rather a tool for subtly conveying his desired message, a message with strong political tones. I will treat these passages in the order in which Julian wrote them, although that places the events reminisced about in the reverse order. (shrink)

The rhetorical career of Libanius of Antioch (a.d.314–c.393) spanned the reigns of a number of fourth-century emperors. Like many orators, he used the trope of the emperor as a pilot, steering the ship of state. He did this for his imperial exemplar Julian and in fact for his predecessor Constantius II as well. Julian sought to craft an identity for himself as a theocratic king. He and his supporters cast him as an earthly parallel to the Christ-like versions of Heracles (...) and Asclepius he constructed, which was arguably a co-opting of Christian and particularly Constantinian themes. In a public oration, Julian even placed himself in the role of Christ in the Temptation in the Wilderness. This kind of overtly Christian metaphor was not Libanius’ preferred idiom, however, and he wrote of Julian as another kind of chosen and divine saviour-figure, one with its roots in the golden age of Greek philosophy. The figure of the κυβερνήτης, the ‘pilot’ or ‘helmsman’, is a philosophical concept with roots in the thought of the pre-Socratics but most familiar from Plato. The uses of this metaphor by Julian and Libanius highlight the rhetorical strategy and self-presentation the emperor employed during his reign. (shrink)

Pragmatism is a distinctive approach to clinical research ethics that can guide bioethicists and members of institutional review boards (IRBs) as they struggle to balance the competing values of promoting medical research and protecting human subjects participating in it. After defining our understanding of pragmatism in the setting of clinical research ethics, we show how a pragmatic approach can provide guidance not only for the day-to-day functioning of the IRB, but also for evaluation of policy standards, such as the one (...) that addresses acceptable risks for healthy children in clinical research trials. We also show how pragmatic considerations might influence the debate about the use of deception in clinical research. Finally, we show how a pragmatic approach, by regarding the promotion of human research and the protection of human subjects as equally important values, helps to break down the false dichotomy between science and ethics in clinical research. The build system thus serves as both a production conversion engine and software test harness. We have now processed the complete arχiv collection through 2006 consisting of more than 400,000 documents (a complete run is a processor-yearsize undertaking), continuously improving our success rate. We are now able to convert more (...) than 90% of these documents to XHTML+MathML. We consider over 60% to be successes, converted with no or minor warnings. While the remaining 30% can also be converted, their quality is doubtful, due to unsupported macros or conversion errors. (shrink)

The use of upper limb vibration during exercise and rehabilitation continues to gain popularity as a modality to improve function and performance. Currently, a lack of knowledge of the pathways being altered during ULV limits its effective implementation. Therefore, the aim of this study was to investigate whether indirect ULV modulates transmission along spinal and corticospinal pathways that control the human forearm. All measures were assessed under CONTROL and ULV conditions while participants maintained a small contraction of the right flexor (...) carpi radialis muscle. To assess spinal pathways, Hoffmann reflexes elicited by stimulation of the median nerve were recorded from FCR with motor response amplitudes matched between conditions. An H-reflex conditioning paradigm was also used to assess changes in presynaptic inhibition by stimulating the superficial radial nerve 37 ms prior to median nerve stimulation. Cutaneous reflexes in FCR elicited by stimulation of the SR nerve at the wrist were also recorded. To assess corticospinal pathways, motor evoked potentials elicited by transcranial magnetic stimulation of the contralateral motor cortex were recorded from the right FCR and biceps brachii. ULV significantly reduced H-reflex amplitude by 15.7% for both conditioned and unconditioned reflexes. Therefore, ULV inhibits cutaneous and H-reflex transmission without influencing corticospinal excitability of the forearm flexors suggesting increased presynaptic inhibition of afferent transmission as a likely mechanism. A general increase in inhibition of spinal pathways with ULV may have important implications for improving rehabilitation for individuals with spasticity. (shrink)

Redish et al. trace vulnerabilities in habit and planning systems almost exclusively to pharmacological effects of addictive substances on underlying brain systems. As we discuss, however, these systems also can be disrupted by purely psychological factors inherent in normal decision-making and everyday behavior. A truly unified model must integrate the contribution of both sets of factors in driving addiction.

The purpose of this study is to assess the relationship between involvement in three aspects of congregational life and spiritual growth. In addition, an effort is made to see if spiritual growth may, in turn, affect health. A latent variable model was developed to test the following hypotheses: individuals who attend worship services more often, attend Bible study and prayer group meetings more frequently, and individuals who receive more spiritual support from fellow church members will be more likely to report (...) they have experienced spiritual growth; people who experience more spiritual growth will be more likely to derive a deeper sense of meaning in life; and individuals who have developed a deeper sense of meaning in life will enjoy better health. Data from the U.S. Congregational Life Survey suggests that spiritual support has the stronger relationship with spiritual growth than either worship service attendance or involvement in formal church groups. The findings further reveal that greater spiritual growth is associated with a deeper sense of meaning in life and meaning, in turn, is associated with better health. (shrink)
